Improving Diabetes Management: Physician Practices

There are two tracks towards obtaining the diabetes award. You only need to choose and follow one track and meet its achievement criteria in order to win the award for diabetes.

Improving Diabetes Management (Track 1)
Achievement Criteria

 

  • Host a Diabetes Personal Action Toward Health (PATH) workshop for at least five Medicare Fee-for-Service (FFS)/dual eligible patients with diabetes*

  • Submit clinical outcome data for at least five Medicare beneficiaries completing provider hosted PATH workshop*

  • Submit intervention form related to one measure of your choice (HbA1c, blood pressure control, cholesterol management, foot exam, eye exam, weight control)

  • The intervention form should detail how you will work to improve clinical outcomes of HbA1c, lipids, blood pressure or weight control OR increase adherence to clinical guidelines for appropriate use of utilization measures for HbA1c, lipids and eye exams among your Medicare patient population.

  • Submit data for your chosen intervention measure (HbA1c, blood pressure control, cholesterol management, foot exam, eye exam, weight control)

  • Achieve the evaluation performance standard of five percent absolute improvement from baseline for chosen intervention measure

  • Submit final Governor's Award of Excellence report (narrative)

*To be completed at some point between July 31, 2017 and Dec. 15, 2018

  • Submit data for your chosen intervention measure

Dec. 15, 2018

  • Submit data for your chosen intervention measure

    • Time period: 10/1/17-9/30/18

  • Achieve the evaluation performance standard of 5 percent absolute improvement from baseline for chosen intervention measure

    • Baseline: 7/1/16-6/30/17

    • Evaluation time period: 10/1/17-9/30/18

  • ​Ensure you have submitted PATH workshop attendees and clinical outcomes

  • Submit final Governor's Award of Excellence report (narrative)

Improving Diabetes Management (Track 2)
Achievement Criteria

 

  • Submit intervention form related to one measure of your choice (HbA1c, blood pressure control, cholesterol management, foot exam, eye exam, weight control)

  • The intervention form should detail how you will work to improve clinical outcomes of HbA1c, lipids, blood pressure or weight control OR increase adherence to clinical guidelines for appropriate use of utilization measures for HbA1c, lipids and eye exams among your Medicare patient population.

  • Submit data for your chosen intervention measure (HbA1c, blood pressure control, cholesterol management, foot exam, eye exam, weight control)

  • Submit written documentation of a point of care referral process for Diabetes PATH*

  • Identify patients with diabetes through your EHR and refer 20 percent of those patients to a Diabetes PATH workshop*

  • Achieve the evaluation performance standard of five percent absolute improvement from baseline for chosen intervention measure

  • Submit final Governor's Award of Excellence report (narrative)

*To be completed at some point between July 31, 2017 and Dec. 15, 2018
