2014 Governor's Award of Excellence

Achievement Criteria

 

  • Register and identify community champion(s)

  • Submit all required Governor’s Award of Excellence documentation

  • Sign and submit a community charter including providers from at least three levels of care (can include community-based organizations)

  • Select and implement three interventions (from list below) to improve coordination of care and demonstrate positive trending in at least one of three (interventions can be from different organizations)

    • Screen and reduce adverse drug events (required)

    • Improve quality of care and outcomes in post-acute care settings

    • Reduce readmissions in acute care settings

    • Improve provider, patient and caregiver communication in ambulatory settings

  • Submit quarterly intervention data

  • Acute care hospital providers must achieve the following:

    • 3 percent relative improvement rate (RIR) from baseline in acute care admission and 6 percent RIR from baseline in acute care readmission rate

    • AND at least one of the following:

      • 6 percent RIR in skilled nursing facility from baseline readmission rate

      • 6 percent RIR in home health agency from baseline readmission rate

Note: MPRO will work with each community to determine a mutually agreed upon number of providers per care setting

Performance Periods

 

Aug. 28, 2017

  • Register and identify community champion(s)

  • Provide a signed community charter within three months of Governor's Award of Excellence application

  • Select interventions

 

Oct. 15, 2017

  • Submit intervention effectiveness measure form for three interventions – one intervention must address medication safety for one of three high-risk medication categories: anticoagulants, diabetic agents, and/or opioids

    • The intervention form should detail how you will work to improve coordination of care and quality of care to reduce admission and readmission and increase community tenure among your Medicare patient population

  • Submit quarterly intervention data for three measures for baseline time period 7/1/16-6/30/17

 

Jan. 15, 2018

  • Submit quarterly intervention data for three measures for time period 10/1/16-9/30/17

 

Apr. 15, 2018

  • Submit quarterly intervention data for three measures for time period 1/1/17-12/31/17

 

Jul. 15, 2018

  • Submit quarterly intervention data for three measures for time period 4/1/17-3/31/18 

 

Dec. 15, 2018

  • Submit quarterly intervention data for three measures

  • Submit final Governor’s Award of Excellence narrative detailing how your organization has fostered innovation in healthcare quality improvement and facilitated rapid adoption within and across organizations and communities
