Care Coordination
MPRO has vast experience in large-scale community-based care coordination programs that improve care transitions, reduce readmissions, improve the patient experience and help drive a positive impact on costs related to care. As the state of Michigan’s recognized leader in this area, MPRO has assisted in the development, activation and sustainability of care communities that include a widespread group of stakeholders and partners including hospitals, nursing home facilities, home health agencies, clinicians, hospice, palliative care and community-based organizations. Working together, these groups promote mutual use and adoption of best practices as they relate to clinical care, care coordination and community integration.

May Leonard, R.N., BSN, MSBA, CPHQ
May Leonard is the senior vice president & chief operations officer and provides oversight and direction for the Centers for Medicare & Medicaid Services (CMS) work. She has extensive experience in healthcare quality improvement, care coordination, process redesign, culture change, contract compliance and managed care. Ms. Leonard also serves as a key MPRO contact to facilitate collaboration between local, regional and national partners and stakeholders.
