About Our Project
MPRO's Transitioning Care for Patients Receiving Anticoagulant Therapy is a special study designed to reduce patient harm associated with anticoagulant therapy across healthcare settings by:
- Implementing an effective monitoring system for patients receiving anticoagulant therapy
- Identifying and implementing a complete standardized transfer/discharge documentation for the receiving facility
- Empowering patient/caregiver to assume responsibility for their care
