If you would like to submit a written complaint, download and complete the Complaint Letter, and mail the form to MPRO at 22670 Haggerty Rd., Suite 100, Farmington Hills MI, 48335 or fax the form to 1 (248) 465-7431. You may also call MPRO's beneficiary helpline at 1 (800) 365-5899 for more information.
