Engaging Communities in Partnerships to Reduce Adverse Drug Events

 

Background

 

There is a growing awareness that medication errors or Adverse Drug Events (ADEs) are a major source of potentially preventable patient harm. In fact, 1.5 million people are injured each year as a result of medication at an estimated cost of $240 billion per year. ADEs also disproportionately affect patients over the age of 65 across all settings of care including hospitals, ambulatory care and long-term care facilities.

 

ADE Initiative Overview

 

MPRO, (Michigan’s Quality Improvement Organization (QIO), on behalf of the Centers for Medicare & Medicaid Services (CMS) is bringing together hospitals, emergency departments, home health agencies, skilled nursing facilities, physician offices, community-based pharmacies, schools of pharmacy, community health centers and state health organizations to participate in the Patient Safety and Clinical Pharmacy Services Collaborative (PSPC). The PSPC will build a healthcare safety net for older patients with multiple chronic conditions who are at risk for potentially negative consequences from taking multiple medications. The initiative is modeled on the Health Resources and Services Administration’s successful Patient Safety and Clinical Pharmacy Services (PSPC) Collaborative.

 

Participants can expect to:

  • Benefit from participation in a statewide Learning and Action Network (LAN), or a collaborative network of providers sharing best practices;
  • Gain access to evidence-based tools for assessing pharmacy processes and implementing safer practices;
  • Have support for rapid-cycle improvement; and
  • Learn strategies for spreading success within their community.
